Job Summary
The Registered Nurse-Hospice Admissions educates potential patient/families in the Hospice at Home approach to end-of-life care and evaluates their overall readiness and appropriateness for hospice care. The RN visits patients who have been referred for hospice services and conducts a comprehensive examination to determine medical eligibility. The RN will collaborate with the physician to determine eligibility and approve admission to services. The RN will create the initial plan of care to meet immediate patient and family needs and will collaborate with the case manager as needed to ensure a smooth transition into hospice services. The focus of this care is on relief of any pain or symptoms and on caregiver support in caring for the patient until seen by the RN Case Manager. Demonstrates personal accountability for own continuous growth and development. Seeks out appropriate opportunities to enhance competency and improve performance.

? Assesses, plans, implements and coordinates the initial strategies and services required to meet a patient/family?s end-of-life care needs.
? Focuses care plan on relief of any pain or symptoms and on caregiver support in caring for the patient until seen by the RN Care Manager.
? Documents the patient history, diagnosis, medications and plan of care in electronic health record during the admission visit and completes any additional documentation immediately after the visit.

Bachelors: Nursing (Required)
BCLS Certification - AHA, Drivers License - State of Residence, RN License - State of Michigan

? Environmental Conditions: Potential exposure to fumes, dust, gases, and odors. Frequent changes in temperature may occur. Possibility of exposure to communicable diseases.

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
