Job Summary
In collaboration with the entire interdisciplinary team, assess, plan, implement, coordinate, monitor and evaluate the
strategies and services required to meet a patient/family?s end-of-life care needs. An outcome-based plan of care is
created and continually adjusted through utilization of the nursing process, relationship center care principles and holistic
philosophy assuring the changing needs of the patient/family are met while facilitating a pain-free, peaceful, sacred death.
Demonstrate personal accountability for own continuous growth and development. Seek out appropriate opportunities to
enhance competency and improve performance.

Provide the care that patients? want, when they want it.
? Develop a comprehensive plan of care that addresses physical, emotional, cultural and spiritual needs of the
patient/family with input from the patient/family and the interdisciplinary team. Evaluate this plan and makes interventions
as necessary.
? Advocate for and ensures pain/symptom management. Educates and provides the support to patient/family regarding
care issues.
? Direct and monitor the bedside care of patients provided by RN, LPN and CAN staff at the residence. Assist with direct
patient care during times when more staffing is needed.
? Administer, document and oversee medications/treatments as ordered by the physician according to policies in an
accurate and timely manner.
? Identify and respond appropriately to emergency situations.
? Attend IDT meetings to facilitate coloration with clinical team members regarding the patient care plan.
? Complete admissions when needed.
? Visit external partners to educate on end of life care at the residence.
? Collaborate effectively and professionally with both internal and external partners. Attend IDT meetings to facilitate
collaboration with clinical team members regarding the patient plan of care.
? Document and ensure documentation in the patient plan of care, planned interventions, medications and evidence of
decline in electronic health record during the visit and completes any additional documentation immediately after the visit.
? Ensure compliance with all federal, state, local, and CHAP requirements.
? Perform other duties as assigned.

Bachelors: Nursing (Required)
BCLS Certification - AHA, Drivers License - State of Residence, RN License - State of Michigan

? Skills: Basic computer, data entry, typing, filing and organization skills required. Critical thinking, proficient in EMR, proficient with pain and symptom management, high level of rapport.
? Abilities: Ability to work extended hours beyond normal work schedule to include, but not limited to, evenings, weekends, extended shifts and/or extra shifts, various locations, sometimes on short notice.
? Environmental Conditions: Potential exposure to fumes, dust, gases, and odors. Frequent changes in temperature may occur. Possibility of exposure to communicable diseases.

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
